Kogi State Government - Invitation to Tender for Supply of Equipment and Facilities for Phase 2 of Kogi State Comprehensive Medical Diagnostic Centre

The Kogi State Tenders Board is inviting interested reputable contractors for Bid to tender for:

1.) The Supply of Equipment and Facilities for Phase 2 of Kogi State Comprehensive Medical Diagnostic Centre
Kogi State Government desires the equipment for Phase 2 of a state-of-the-art Comprehensive Diagnostic Laboratory and Imaging Centre in Lokoja. This centre is to contain equipment for diagnosing virtually all ailments that will, compete and function like those in advanced countries. Tender must demonstrate a strong background in health information technology and the capacity to provide local and international training of professional staff. Bidders must have a track record for executing projects of this magnitude. They must be able to provide or partner with an Internationally Certified Medical Software company that would synch with all the Phase 1 of imaging equipment already purchased as well as all the components of Phase 2.

2.) Tender Requirements for the Contractors:

• Certificate of Incorporation with CAC
• Company Profile to include
  • Evidence of similar job executed in the last five (5) years i.e Certificates of practical completion and payment.
  • Audited account for the past three (3) years, List of personnel and equipment.
• 3 years Tax Clearance Certificate
• Evidence of Registration with Kogi State Ministry of Works
• Evidence of Financial Capability duly issued from any capitalized Bank in Nigeria.
• Evidence of traceable address (Not P. O. Box) in Nigeria
• Evidence of payment of non-refundable fee of N50,000.00 (Fifty Thousand Naira) for the purchase of the bidding document
• Bid closes three weeks from date of this advertisement.

3.) Collection and Submission of Tender Documents
Interested bidders are to obtain tender documents from the Tenders Board, in the Office of the Secretary to Kogi State Government.

All completed tender documents should be Waxed, Sealed and Submitted to:

4.) Closing Date
All Tenders must be submitted by 12.noon on or before 12th March, 2013.
