Invitation for Pre-Qualification to Tender for Year 2013 Capital Projects at Federal Neuro-Psychiatric Hospital

Preamble
Applications are hereby invited from qualified contractors/companies to tender for pre-qualification for the following projects which will be funded through 2013 Capital Appropriation.

The projects are as follows:
Lot 1: Completion of the Construction of Kitchen and Laundry building at the hospital permanent site;
Lot 2: Completion of the Construction of Male Ward Il (storey) building at the permanent site;
Lot 3: Handling of Research and Development/Outsourced Services.

Pre-Qualification Requirements
Interested contractors/companies are required to submit the following documents in sequential order as listed:

• Certificate of Registration with the Corporate Affairs Commission (CAC);
• Company Tax Clearance Certificate for the last three (3) successive years (2009, 2010 and 2011);
• Evidence of Value Added Tax Registration and Remittances;
• Evidence of Certificate/Exemption from PENCOM in accordance with the Pension Reform Act (2004) including evidence of remittances of employee pension contributions/deductions;
• Evidence of registration/payment of training contribution in compliance with the statutory provision of the Industrial Training Fund (ITF) Act 2011 as amended;
• A sworn Affidavit that no officer of the Federal Neuro-Psychiatric Hospital Uselu, Benin City, is a former or present Director, or has any pecuniary interest in the bid;
• Evidence of financial capability to execute the project (e.g. current bank statement certified by the company’s banker) and/or banking support;
• Verifiable list of similar jobs successfully executed in the last three (3) years including letters of award of contracts, project costs, job completion certificate etc.;
• Company’s Audited Accounts for 2009, 2010 and 2011;
• Company’s profile including professional qualifications of key personnel and equipment (where applicable);
• Any other additional information that may enhance the company’s application.

Note: Documents (I) (x) are RESPONSIVE criteria, the absence of any automatically disqualifies the bidder from pre-qualification.

Closing Date/Submission of Prequalification Documents

• The completed pre-qualification documents are to be submitted in sealed envelope with the name and address of the prospective bidder at the top left corner of the envelope with tendere’s GSM number/reachable number
• All documents should be submitted within six (6) weeks i.e. on or before 9.00a.m. on or before Monday, 4 March 2013 when documents will be opened;

• Tenderers should note that only pre-qualified companies/contractors would be invited to submit financial bids. The hospital reserves the right to reject any or all pre-qualified documents.

All tenderers and interested stakeholders are invited to the tender opening scheduled for Wednesday, 3’ April 2013 in the hospital’s Conference Hall.
