The Lagos University Teaching Hospital desires to procure drugs for the Pharmacy Department of the Hospital with a view to improving patient care services in the Hospital.
In consideration of the above:
- Invitation for submission of Pre-Qualification documents are being invited from interested reputable companies who wish to tender for the above supply.
- Invitation to tender financial bids for the above supply from reputable companies who pre-qualified in the pre-qualification exercise, will be conducted at a later date.
Submission of Pre-Qualification Documents
Interested companies on the above are to submit “Pre-Qualification Documents” which must include but not limited to:
- Formal application.
- Company’s profile indicating the Management profile including the names and professions of the Top Management Staff.
- 3 Years (Current) income Tax Clearance Certificate.
- 3 Years Company audited Accounts.
- Certificate of Registration and Retention of Premises.
- Bankers reference.
- Current annual Licence to practice as Pharmaceutical Chemist of the Company’s Superintendent Pharmacist.
- Certificate of Incorporation.
- Registration with Pension Administrator.
- Evidence of contribution to Industrial Training Fund.
- Any other relevant information.

“Pre-Qualification Documents for Technical Bid for Supply of Drugs at LUTH” must be submitted in sealed envelope and addressed to:

Closing Date
The Pre-Qualification documents must be dropped in a Tender Box provided in the office of the Director of Administration on
or before 12 noon, 19th April, 2013.
Opening of the Pre-Qualification documents will be on the same date at the new
Board Room at 12:30pm. All interested companies are invited to attend the opening as scheduled above.
Tenderers should note that only Pre-Qualified companies will be invited to submit Financial Bids. The Lagos University Teaching Hospital reserves the right to reject any or all Pre-Qualification documents.
