The office of the Adamawa State MDGS Coordinator invites tenders from interested competent contractors for the following projects under the 2013 approved MDGs – CGS to State and LGAs’ projects;

Invitation for Pre-Qualification for the Millennium Development Goals Conditional Grants Scheme 2013 Projects

• Construction and installation of solar powered Boreholes in 25 locations
• Provision of Small Town Water Schemes in 5 Locations;
• Rehabilitation of PHC Clinics in 9 Locations;
• Construction of PHC Clinics in 13 Locations;
• Supply and installation of solar lighting in 17 PHC Clinics;
• Supply of Essential PHC Medical Equipment to 28 Clinics;
• Construction of 21 VIP Latrines;
• Construction of staff Quarters in 20 PHC Clinics
• Construction and equipping of 2 Standard vaccine cold Room/Store;
• Construction of 1 Comprehensive Health Center
• Construction of 1 standard Dry store;
•  Supply and Installation of 1 Standard Incinerator;
• Construction of Office Block for ADPHCDA

Scope of Work
The project involves the following in line with the approved designs and specifications

• Construction and installation of solar powered boreholes in 25 locations; Geophysical survey and its comprehensive report; borehole design, drilling and casing with 6 inches; 20ltr steel with HA-beams stanchions of 6m high; installation of 1hp solar pumps and 480-520w solar plates on the tank; fencing and landscaping; reticulation up to 500m and water fencing points;
• Provision of small town water schemes in 5 locations  Geophysical surveys and their comprehensive report; boreholes design, drilling and casing with 6 inches; 100ltr steel tanks with H-beams stanchions of 12m high; 250ltrs steel ground tank; installation of 5hp water pumps and 25kva diesel powered generators, office block, installation rooms, fencing and landscaping;
• Rehabilitation of PHC clinics in 9 locations demolition and alterations, roof changing, block works and re-flooring, electrical and mechanical alterations, finishing and painting, clearing and landscaping;
• Construction of PHC clinics in 13 locations construction of new PHC clinics with 2-bedroom semi-detached single quarters according to an existing design and specification;
• Supply and installation of solar lighting in 17 PHC clinics supply and installation of specific items according to an existing and specification;
• Supply of essential PHC medical equipment to 28 clinics and installation of specific items according to an existing and specification;
• Construction of 21 VIP latrines construction of VIP latrines according to existing design and specifications;
• Construction of staff Quarters in 20 PHC clinics construction of staff quarters according to an existing design and specifications;
• Construction and equipping of 2 standard vaccine cold room/store construction and equipping vaccine room/store according to an existing design and specifications;
• Construction of 1 comprehensive health centre construction of comprehensive health centre according to an existing design and specifications;
• Construction of 1 standard dry store construction of dry store according to an existing design and specifications;
• Supply and installation of 1 standard incinerator construction and installation of functional incinerator according to an existing design and specifications;
• Construction of office block for ADPHCDA construction of office block according to an existing design and specifications;

• Each contractor is expected to submit the following mandatory documents:
• Evidence of incorporation with Corporate Affairs Commission (CAC);
• Evidence of Tax clearance Certificate for the last three years (2010, 2011 & 2012);
• Company Audited Account for three years (2010, 2011, & 2012) prepared by an accredited charted accountant(s);
• Liquid and/or evidence of access to or availability if confirmed credit facilities of not less than N10, 000,000.00 from reputable bank;

Other Criteria

• Evidence of Financial capability and banking support (Bank reference, recent statement of account);
• Evidence of “similar project” executed over the last 5 years and evidence of knowledge of the industry (award letters, valuations and completion certificate);
• List of on-going projects (if any);

Submission of Completed Pre-Qualification Documents
The pre-qualification documents should be submitted in sealed envelope and clearly marked “ pre-qualification to tender Adamawa MDGs/CGS/2013” and shall be delivered to:

• Failure to provide any of the above will disqualify the bidder.
• The documents will open at 12:00 noon on the closing date at the training room of the Internet Base Station, Government House Yola.
• No late submission will be entertained.
