Centre for Integrated Health Programs (CIHP) under its Strengthening Skills and Competencies of Care Providers for Enhanced Service Delivery (SCOPE) Project -a Presidents Emergency Plan for Aids Relief (PEPFAR) fit project through the Centre for Disease Control & Prevention (CDC), is working on supporting Resources for the Health program of the Federal Government of Nigeria through development of a web location database for use in Medical & Dental Council of Nigeria.

Objective of the Service:
MDCN have a newly developed web portal with an online database that is capable of professional registration generating routine ad hoc report as required by the council, generate certificates and licenses, Integration with inters witch gateway for online payment of registration and licensing fees; To achieve full automation of the registration process of medical. Dental & alternative Medicine Practitioners in Nigeria. There is need for an upgrade of this web portal to include:

Part A-Web Application development Upgrade:
Development of medical education module; inspectorate Department Module; Professional Discipline module in the portal Supply, Installation and configuration of synchronization servers with all necessary software required Digitize 75000 files and link each file with corresponding unique folio number Training on the administration and management of the IT infrastructures used for the project End user training for the staff of the various departments and zonal offices located in Lagos Kaduna and Enugu.
CIHP Nigeria invitation to reputable IT firms to submit proposal for development of web application including IT Infrastructure upgrade, LAN installation, Internet service Provision and web application database for registration and other information capture of Medical Practitioners in the Country

PART B- Internet Service Provision
Upgrade on the existing 1MB/1MB Vsat Internet to dedicated6MB/6MB Bandwidth on KAB Band.

PART C -IT Infrastructure
Upgrade of the LAN Networking system in the Head office in Abuja and the three regional offices in Lagos, Kaduna & Enugu

PART D-Fire Suppression System & Power Supply
Supply and installation of fire suppression system for the IT infrastructure protection;
Upgrade of the existing inverter system to provide sustainable and uninterrupted power supply

Prequalification Requirements:

• Evidence of company Registration Including but not limited to Certificate of Incorporation in Nigeria with CAC, CAC forms C07 and C02 Head Office address as evidenced in your office utility bill or any other relevant document.
• Company Profile which includes list of key personnel with their curriculum vitae and evidence of registration of these personnel with relevant professional bodies;
• Evidence of VAT registration /Remittance Certificate and income tax clearance certificate for the most recent three years -
• Audited account for the last three Years
• Reference from our bankers as evidence of financial capability
• Evidence of successful execution of similar projects in the past

Submission of Application
At the top right corner of the Prequalification submission envelope should be inscribed the words “Chip Rep-Web Application Database MDCN” and 2 sealed envelopes containing hard copies of the submission should be forwarded to the CIHP office address below:
