4. Building of an office block (Biomedical Engineering)
5. Renovation and upgrading of Pathology Building
6. Supply of equipment to new Born Unit
7. Supply and installation of ventilators (2Nos. Children, 2Nos. Adult s) and Multiparameter Monitors (10Nos.)
8. Procurement of Ambulance (1No.)
9. Supply and installation of Implant Placement and Restoration equipment in Dental Centre
10. Supply and installation of Dicom Printer for Digital X-ray machine
11. Erection of a Service building in Accident & Emergency Complex
12. Supply of Anaesthetic machines (2Nos.)
13. Supply and installation of equipment to Vitro-renal Unit
14. Supply and installation of Thermoscientific Histostal Embedding Workstation
15. Supply and installation of original GA-18 Air Compressor for Oxygen Plant
16. Supply of spare parts for X-ray C-ARM Fluorostar 7900
17. Construction of a Block of Classrooms/Laboratory Complex

Eligibility Criteria
Firms who are interested in bidding for jobs must possess the following eligibility requirements:

1. Certificate of registration with the Corporate Affairs Commission (CAC)
2. Detailed Company Profile
3. Tax Clearance Certificate for the last three (3) years (2011, 2012 and 2013).
4. Evidence of VAT compliance for the last three (3) years including VAT Certificate.
5. Evidence of registration with PENCOM/remittances.
6. Evidence of registration with ITF as well as remittances.
7. Audited company account for the last three (3) years (2011.2012 and 2013).
8. Evidence of similar jobs/assignment completed/on-going by the company in the past.
9. Evidence of financial strength and credit-worthiness and solvency from company’s Banker(s).
10. A sworn affidavit in line with the provisions of Part IV, Section 16, Subsection 6(f) of the Public Procurement Act, 2007 stating that:

• All documents/information presented are genuine and verifiable
• The Director(s) o(the firm has/have never been convicted by any court of law within oroutside Nigeria
• None of the company’s Directors or the company itself is bankrupt
• None of the officers of University of Benin Teaching Hospital (UBTH) or Bureau of Public Procurement (BPP) is a former or present staff of the company
• The firm is not in receivership.

These documents are to be arranged in the order in which they are listed.

Collection of Bidding Documents
Interested companies are to collect the Tender Documents from the Procurement Division (Room 8) Administrative Block, (Laundry Extension) from the date of publication of this advert between the Hours of 9:00am and 4:00pm daily with the evidence of payment of a non-refundable fee of N30, 000.00 (Thirty thousand naira) only per lot. The sum is to be paid into UBTH Account number 1770413127 with Skye Bank Plc, UBTH premises, Benin City and a photocopy of the receipt presented before the collection of Tender documents.

Submission of Bidding Documents
Tender documents are to be enclosed in two separate envelopes, one to contain the Technical Bid which must be boldly marked TECHNICAL BID” on the top right corner of the envelope, while the second envelope is to contain the Financial bid which is also to be boldly marked ‘FINANCIAL BID on the top right corner of the envelope. The two envelopes are to be placed in one large envelope which should be addressed to:

The lot number of the job bidded for should be written on the top right corner of the front of the envelope. Also the name of the firm should be written boldly on the other side of the envelope.

The envelopes are to be submitted in a Tender box at the Procurement Division (Room 12) Administrative Block (Laundry Extension) after they have been registered. All Tender documents are to be submitted not later than 12:00noon on Tuesday 2nd September 2014.

All submissions are to be made in English Language.

Opening of Bidding Documents
The bids shall be opened immediately following the deadline for submission on Tuesday 2nd September, 2014, by 12:00 noon at the VIP Lounge, Catering Department, UBTH, Ugbowo, Benin City. Bidders or their representatives as well as interested members of the public are invited to witness. the opening of the bids.
