Invitation to Tender for the 2014 Draft Statutory Budget at University College Hospital, Ibadan

The University College Hospital, Ibadan, Nigeria’s foremost teaching Hospital have provision in the 2014 Draft Statutory Budget and intend to use part or whole of it for the procurement described hereunder. Competent and reputable contractors, suppliers and manufacturers with relevant experience and good track record are hereby invited to tender under the following scope/schedule:

Scope

Description
LOT 1: Purchase of Furniture for the Snake-bite Centre, CHO Building, Canteen and Hospital Quarters at the University College Hospital, Ibadan Comprehensive Health Centre, Sepeteri, Oke-Ogun, Oyo State.

LOT 2: Supply of Drugs, Vaccines, Laboratory Equipment & Consumables, Solar Powered Freezer and Solar Powered Fridge for use at the University College Hospital, Ibadan Comprehensive Health Centre, Sepeteri, Oke-Ogun, Oyo State.

LOT 3: Supply of Brand new Vehicles

LOT 4: Renovation and Rehabilitation of House Officers Residence (Resident Doctors Hostel) at the University College Hospital, Ibadan.

Method of Application
Interested Contractors, Suppliers or Manufacturers should submit along side their Tender, five (5 Nos) copies of their detailed proposals with the following, as these will be used for prequalification. A comprehensive and paged table of contents, providing information on the company profile among other requirements should be presented in the following order:

1. Letter of application
2. Certificate of registration with the Corporate Affairs Commission (CAC)
3. Copies of immediate past three (3) years Tax Clearance Certificate with Tax Identification Number (TIN)
4. Evidence of three (3) years Company’s Audited Accounts
5. List of key personnel including their professional registration certificate, their CVs with
6. passport photograph endorsed by the company Chief Executive (for Companies bidding for LOT4)
7. Listof key equipment (applicable to LOT4);
8. Evidence of Financial Capability and Bank Support;
9. Evidence of VAT registration and remittance;
10. Verifiable evidence of at least (5 Nos.) similar projects handled in the past 5 years (indicating the clients, contract value and location of the projects with evidence of completion certificates or IPCs, if on-going);
11. Evidence of compliance with the ITF Amendment Act, 2011 by inclusion of compliance letter from the Industrial Training Fund;
12. Evidence of compliance with Pension Reform Act, 2004 by inclusion of letter of compliance letter from the National Pension Commission;
13. Manufacturers’ Authorization (Applicable to LOT 1-3) m.) A letter authorizing University College Hospital, Ibadan to verify claims with relevant agencies and bodies;
14. Sworn Affidavit in line with the provisions of Part IV, Section 16, subsection 6(f) of the Public Procurement Act, 2007, which should include among others that:

• all information presented are true and correct in all particulars;
• that the Director(s) of the firm has/have never been convicted by any Court of Law;
• that none of the company’s director or the company is bankrupt;
• that none of the officers of UCH, Ibadan or BPP is a former or present director/staff of the company;
• that the firm is not in receivership

Method of Paying For Tender Documents
Interested suppliers/contractors should pay a non-refundable tender fee of N20, 000.00 (Twenty Thousand Naira) only into the Hospital’s Tender Account at the First City Monument Bank (FCMB) Plc, UCH Branch and convert it to the Hospital’s receipt at the Finance & Accounts Department of the Hospital. Photocopy of the receipt should be attached to the Tender Documents.

Submission of Tender Documents
Submission of documents is to be done in two separate envelopes clearly marked at the top left corner “Proposal/Technical Bid (of the applicable LOT) FOR YEAR 2014 Projects” and “Financial Bid (of the applicable LOT) for year 2014 with the company’s name at the bottom left corner for easy identification. The completed documents should be enclosed in an envelope, sealed and marked appropriately in Capital Letters at the Left Corner and addressed to:

The completed and sealed tender documents should be deposited in the Tenders Box in the office of the Deputy Director (Procurement) not later than 12:00 noon on Tuesday 23rd September 2014 and any submission made thereafter stands rejected.

Opening
The submitted documents will be opened on the same day Tuesday 23rd September 2014 at 12:00noon at the Omolade Alade Hall of the School of Nursing, UCH, lbadan. Bidders are required to be present at the opening.
