1. Evidence of Company Registration with Corporate Affairs Commission
2. Evidence of Registration with the College of Health Sciences and Technology, ijero-Ekiti.
3. Evidence of Registration with Bureau of Publlc Procurement of Ekiti State
4. Evidence of Company’s Current Tax Clearance Certificate for the last three consecutive years.
5. Evidence of payment of Development Levy in Ekiti State for the last three years.
6. Evidence of Business premises Registration in Ekiti State
7. Evidence of Financial capabilIty of Bank support
8. Company’s audited account for the last three years.
9. Evidence of payment of a non-refundable fees of a (N150, 000) only
10. Evidence of the execution of previous works similar in nature
11. Comprehensive company profile.
