Request For Proposal on Provision of Comprehensive Health Management Services to Family Health International (FHI 360)

The Nigeria Country Office hereby invites suitably qualified Health Maintenance Organizations (HMOs) to send in proposals for the provision and management of healthcare services to our employees as per the scope below.

Scope of Work
The organization will be expected to provide high quality and cost effective healthcare services to employees of FHI 360 Nigeria. The selected organization is expected to ensure easy access to healthcare services across Nigeria at all times. The services to be provided will include, but are not limited to the following:

• Provision of comprehensive high quality healthcare services to our staff 24 hours 7 days of the week covering areas such as: general medical services, dental services, laboratory services, gynaecological services, ophthalmological services, paediatric services and surgeries .covered amongst others.
• Prompt emergency response and evacuation should the need arise
• High quality referral systems among health providers
• Provision of usage statistics to FHI 360 Management
• Advisory services to FHI 360 on healthcare services such as wellness programs

Requirements
Proposals should be submitted with the underlisted documents:

1. Company profile
2. Certificate of Incorporation
3. Evidence of current accreditation by the National Health Insurance Scheme (NHIS>
4. List of providers under the scheme
5. Verifiable list of current clients, including the NGO sector
6. Types of plans and coverage provided under each category
7. List of exclusions
8. Proposed fees per plan/scheme
9. Number of personnel and regional offices
10. Tax Clearance Certificate for year 2013. 2012, and 2011
