Request for Expression of Interest for the Lease of Hospitals Mile 18 Site at Igue-Yeye Village at the Federal Neuro Psychiatric Hospital, Benin City

The Federal Neuro Psychiatric Hospital, Benin City, Edo State with a view to boosting revenue and operational efficiency through Public Private Partnership Initiative (PPPI), is requesting for suitably qualified companies to submit relevant documents for the lease of some of the profitable units in the hospital.

Request for Expression of Interest for the Lease of Hospitals Mile 18 Site at Igue-Yeye Village 

Consequently, in accordance with international best practices, the hospital hereby invites suitably qualified companies to submit documents for the under listed:

Lot 1: Lease of Hospital Site at Mile 18 Igue-Yeye Village

Interested companies are required to submitihe following documents;

1. Company Tax Clearance for the last three (3) years.
2. Evidence of Company Registration with Corporate Affairs Commission.
3. Verifiable list of similar job successfully undertaken.
4. Company financial details/Bank Statement
5. Value Added Tax Certificate (VAT)
6. List of key staff and their Positions.
7. Sworn Affidavit that hospital staff are not directors in the company.
8. Evidence of compliance and remittance of Pension.
9. Evidence of compliance with Industrial Training Fund.

8th December 2014

Submission of Documents
All relevant documents should be submitted in the tender box provided in the OFFICE OF THE MEDICAL DIRECTOR, FEDERAL NEURO-PSYCHIATRIC HOSPITAL, P.M.B. 1108, BENIN CITV EDO STATE in a sealed envelope and labeled Project Name” with Lot number at the top right hand corner of the envelope. Failure to provide any of the listed requirements may result in disqualification/rejection of documents.
The Name, Address, E-Mail and Contract Phone Number of the company must also be written clearly at the back of the envelopes. Submission ends 8th December 2014 at 12:00.noon. Late submissions will not be accepted.

Opening of Submitted Documents
Submitted documents shall be opened immediately after the deadline for submission at 12.00 noon, 8th December 2014 at THE CONFERENCE HALL, FEDERAL NEURO-PSYCHIATRIC HOSPITAL, P.M.B. 1108, BENIN CITY EDO STATE.
