Contractors Needed for the Procurements of Essential Drugs and Medical Consumables/Consultancy Services Under Various Programmes of Kaduna State

Kaduna State Drugs and Medical Supplies Management Agency (the Agency) is an Agency of Kaduna State Government with mandates to manufacture, procure/source, store, sell and distribute quality drugs and medical consumables to both public and private health institutions throughout the state.

Kaduna State Government, through the Agency, hereby invites eligible, interested and competent firms to tender for the supply of drugs and medical consumables under its various Projects/Programmes.

Scope of Works / Description of Projects

1. Procurement of Drugs for Infant Healthcare(IHC) under Free Maternal and Child Healthcare(FNCH) Program -Lot1 (IHC-FMcH)
2. Procurement of Drugs for Under-five Health care(U-5 HC) under Free Maternal and Child Healthcare(iMCH) Program -Lot 2 (U-5HC-FMCH)
3. Procurement of Drugs Under War Against Malaria(WAM) program -Lot 3 (Injections- WAM), Lot 4 (Kits- WAM), Lot 5 (ACTs- WAM), Lot6 (SPs-WAM)
4. Procurement of Antiretroviral Drugs Under HIV/AIDS Treatment Program -Lot 7 (ARVDs)
5. Procurement of Family Planning/Reproductive Health commodities and Consumables -Lot 8 (RI/HTSP Consumables)
6. Procurement of Therapeutic Foods and Drugs for Management of Acute Malnutition -Lot9(RUSF)
7. Feasibility Studies for DMA to Manufacture Essential Medicine/Pharmaceutical (Consultancy) -Lot 10 (Feasibility Studies)
8. Business Plan/Research for DMA to Manufacture Essential Medicines/Pharmaceuticals(Consultancy) -Lot11(Business Plan)

Pre-qualification Requirements

1. Certificate of Incorporation with Corporate Affairs Commission(CAC) including Articles of Memorandum(Form C07) and Associations(Form C02)
2. Evidence of Current Tax Clearance for three years (2014, 2015 and 2016), with Tax Identification Number(TIN) clearly visible
3. Current Certificate of Registration with Kaduna State Tenders Board
4. Current Registration of Business premises with Kaduna State Ministry of Commerce
5. A valid Pharmacists Council of Nigeria (PCN) registration of Premises or evidence of current registration for the year 2017, except for Lot 8, 10 and 11
6. A valid Pharmacist License to Practice or evidence of current registration for the year 2017, except for Lot 8, 10 and 11
7. Evidence of Company's Audited Account for the last three(3) years (2014, 2015 and 2016)
8. VAT Registration and Evidence of past VAT remittance at least In the three years (2014, 2015 and 2016)
9. Company Audited Accounts for three years (2014, 2015 and 2016)
10. Evidence of Financial Capability and/or Banking Support
11. Company Profile, including Official Contact Addresses, Phone numbers, email addresses, and list of key Technical Personnel with their qualification experience
12. Evidence of completion/Execution of other similar verifiable projects within the last three (3) years (2014,2015 and 2016)
13. Any other relevant information that would be helpful in determining the Company's suitability for the Project(s)
14. Verifiable Kaduna State Sub-Treasury Receipt for the payment of Non- refundable Bidding Fee of N50,000.00 in favour of Kaduna State Government for each lot. Submissions not accompanied with the Kaduna State Sub-Treasury Receipt of N 50,000.00 will not be considered.

Submission of Tender
Documents must be made in Six (6) copies (One original and five Photocopies) in a sealed envelope clearly marked with the-name of the Project and Lot Number for which application is made.
Multiple application is allowed but each Lot must be treated on its merit as a separate entity
Tender documents should be submitted not later than 12:00 Noon Nigerian Time on Friday, 28^th of luly 2017 to the office below:

All submissions will be opened immediately after the deadline, on Friday, 28^th of July 2017 at 12.10pm Nigerian Time at the aibove stated office, in the presence of all interested firms that choose to attend.
