The National Orthopaedic Hospital, lgbobi-Lagos invites applications from reputable companies to tender for the supply of the following:

Lot1: Furniture/Office Equipment( Tables, Chairs, Filing Cabinets, Refrigerators, Air- Conditioners and Complete Computer System)
Lot2: Supply of 40KVA Generating Set.
Lot3: Supply of Utility Vehicles.
Lot4: Supply of Medical Equipment:
(a)X-Ray Viewers (Single and Double);
(b) Linen Services Department- Laundry Dryer (Commercial capacity);
(c) Physiotherapy Department- (i) Infra Red Lamp, (ii) Electrical Stimulator, (iii) Adult Life Gait Machine ;
(d)Anaesthesia Department- (i) Patient Monitor, (5 and 7 Parameters), (ii) Video Laryngoscope for difficult airways;
(e) Pathology Department - (i) CX23 Olympus Binocular Light Microscope, (ii) Chemical Auto Analyzer, (iii) 5-Part Hematology Auto Analyzer;
(f) Prosthetic & Orthotics Department-(i) Otto bock Pneumatic SIT Casting Apparatus (743C710), (ii) Otto bock Casting Apparatus, (743A11);
(g) Radio-Diagnosis Department - Digitizer Machine with Cassettes;
(h) Occupational Therapy Department - (i) Bolster Vestibular Swing System, (ii) Multi-functional Upper Extremity work station;
(I) Speech Therapy Department- (i) Dictaphone (Model Sony-lCD), (ii)Visi-Pitch IV (4-39508).
Lot5: Maintenance of Air-Conditioners in the Hospital.

Requirements:

• Evidence of incorporation with the Corporate Affairs Commission (C.A.C).
• Company's profile.
• Three (3) years Tax Clearance Certificate.
• Evidence of payment of VAT.
• Evidence of compliance with Industrial Training Fund (I.T.F).
• Evidence of Registration with Bureau of Public Procurement.
• Evidence of compliance with Pension Act.
• Evidence of three (3) years Audited Accounts of the Company.
• Evidence of Financial capability to handle the job and bank support.
• Evidence of verifiable relevant experience in executing a similar project, giving scope, locations, value and schedule.
  

Submission:
Interested companies should forward six (6) copies of the application (containing requirements) in a sealed envelope and addressed to:

Lagos State.

The completed document should be submitted by hand in the Tenders Box at the office of the Ag. Head of Procurement between the hours of 8:00am - 4:00pm within two (2) weeks of the date of publication of this advertisement (i.e 20^th July - 2^nd August, 2017) upon the payment of Twenty Thousand Naira (N20,000.00) only processing fee per lot. The submission will be closed by 4:00pm on Wednesday, 2^nd August, 2017.

Only shortlisted companies shall be contacted to collect the detailed specifications from Procurement Department for financial bid which will close four (4) weeks after (i.e 29^th August, 2017).
The bid would be opened on Wednesday, 30August, 2017 in the Board Room by 10:00a.m and Bidders or their representatives are required to be present at the event.
