The National Obstetric Fistula Centre, Abakaliki, Ebonyi State proposes to execute the project under 2017 budget, therefore suppliers/service providers to submit documents for the execution of the projects below.

Category A ( Goods)
Lot3: Supply/Installation of Physiotherapy Equipment
Lot5: Supply/Installation of X-ray machine, UPS/ Radiology Accessories
Lot6: Supply/Installation of Office Furniture and fittings
Lot7: Supply of Office Equipment.

Category B (Consultancy Service)
Lot8: Provision of Security Services
Lot9: Provision of Cleaning Services
Lot10: Provision of Catering services

Eligibility/Mandatory Requirements

• Certificate of incorporation from Corporate Affairs Commission
• Certificate of Compliance from PENCOM in accordance with the Pension Reform Act 2004.
• Tax clearance certificate for the immediate past three years (2014,2015 & 2016).
• Certificate of compliance with Industrial Training Fund(ITF) Act, 2001.
• Interim Registration Report (IRR) from Bureau of Public Procurement( BPP)

Other Requirements
Manufacture's authorization from the approved Manufacturers listed the policy for the procurement of medical equipment where applicable.

• Company Audited Accounts for the past Three years (2014, 2015 & 2016)
• Company profile containing list of key personnel's and ownership or availability of equipment's where applicable.
• Completion certificate for three(3) similar jobs successfully executed within the last three years.
• Reference letter from reputable Banks with Banks statements of Account(Jan-Dec.2016)/evidence of availability of credit facilities (Not less than 50 percent of quoted price ) from a reputed Bank
• Affidavits declaring the originality of the Documents.

Bids Collection
Bids documents can be obtained from the Office of Secretary Tenders Board. Room 04 Admin Block with of a copy and original receipt of non - refundable Tender Fee N10,000.00 ( Ten Thousand Naira) paid National Obstetric Fistula Centre Abakaliki(Remita Account) for each lot.

Submission of Bid Documents
Completed Technical and Financial Bids ( Three sets of hard copy and 1 soft copy ) shall be submitted in different sealed temper proof envelop and labeled " Technical Proposal " & Financial Proposal" respectively and both placed on third envelop . All the 3Nos, envelop should bear the "Project Name " and Lot No at the Top Right Hand Corner of the Envelop ,  Company Name at the reverse side and addressed to :

National Obstetric Fistula Centre, Abakaliki,
86 Abakaliki-Enugu Expressway, Abakaliki,
Ebonyi State.

The Documents should be deposited in Tender Box at the Admin Complex of National Obstetric Fistula Centre, Abakaliki.

Closing Date and Time For Submission Of Documents
Closing date/time for all submission shall be 12 noon prompt on Monday 4^th September, 2017.

Bid Opening
Technical( Bid Documents shall be opened in Public immediately following the deadline of the submission of bids at the Admin Complex of National Obstetric fistula Centre, Abakaliki,
