Invitation for Pre-Qualification to Tender for Capital Projects at the Ahmadu Bello University Teaching Hospital Permanent Site, Shika Zaria

The Ahmadu Bello University Teaching Hospital invites interested and reputable companies/contractors with requisite experience and competence to submit prequalification documents in respect of the 2017 capital projects execution in the hospital.

Scope of Works
Lot 1: Procurement of Medical Equipment and Instruments to replace the old/outdated ones.
Lot 2: Equipping of Day Care Theatre.
Lot 3: Construction and Furnishing of 4B Apartments for staff on call duties
Lot 4: Free Medical Mission (treatment and surgery) in Sabon Gari Federal Constituency Kaduna State.

Pre-Qualification Criteria
Interested companies are required to Submit the following documents and present original copies for sighting during the bid opening:

• Verifiable Evidence of Incorporation and registration with Corporate Affairs Commission.
• Verifiable Evidence of Registration with BPP National Database of contractors by inclusion of current Interim Registration Report (IRR).
• Tax Clearance Certificate verifiable from Federal Inland Revenue Services (FIRS) for the last 3year's (2014 2015 & 20l6)expiring December 2017.
• Company Audited Account for the last 3 years (2014,2015 & 2016).
• Verifiable Evidence of VAT Registration with Tax Identification Number (TIN) and up-to-date evidence of remittance.
• Verifiable Evidence of compliance with the Pension Reform Act 2014 by inclusion of current certificate of compliance from the National Pension-Commission.
• Verifiable Evidence of compliance with the industrial Training Fund (ITF) Amend Act 2011 by inclusion of current compliance letter from ITF.
• Verifiable Evidence of compliance with the Nigeria Social Insurance Trust Fund (Employee Compensation Act 2010) by inclusion of current compliance Certificate from NSITF.
• Manufacturer Authorization in respect of Medical Equipment and Instruments.

Selective Criteria
In addition to the above, bidders should also submit the following:

• Evidence of Financial capability supported with bank reference.
• List of similar, verifiable and successfully executed projects/ongoing by the company with letters of award and completion certificates. Only letters of award and interim certificate of payment should be attached for projects that are ongoing.
• Company profile including list of professional staff qualification which should include names, Cv's and copies of certificate of each staff.
• list of equipment's owned or lease hold by the company relevant for smooth execution of the project.
• Every Submission must be accompanied with a sworn affidavit that all information and documents submitted are true and correct and that no officer of the commission is a former or present director, shareholder or someone with any pecuniary interest in the bidding.

Submission of Pre-Qualification Documents
Application for Pre-qualification should be submitted in a sealed envelope marked "Prequalification Tender". Please indicate the Lot Number and project title at the right hand corner of the envelop and indicating bidders name and address an the reverse side and addressed to:

Kaduna State.

The Tender Documents should be hand-dropped in the Tenders Box situated at the office of the Chief Medical Director, ABUTH Zaria, not later than 3weeks from the date of this publication i.e. on or before 12:00 Noon of Monday August 21^st, 2017.

Opening of Tender Documents
The Pre-qualification documents will be opened on the same day in the presence of applicants or their representatives by 1:30pm, of Monday August 21^st, 2017, at the Board room of Ahmadu Bello University Teaching Hospital Shika Zaria.

• late submission shall be rejected.
• This notice is not an invitation to tender for the job.
• This advertisement is published for information purposes Only and should not be considered as a commitment or obligation on the part of the Hospital to award any contract/Supplier or Agent.
• Only companies who meet the above stated prequalification requirements would be contacted to tender.
• Due diligence should be followed as all documents submitted would be verified. Past. executed works and supplier/contractor offices may be visited.
• Failure to comply with any of the above stated instructions or to provide any of the listed documents may automatically result in disqualification.
• Any discrepancy found would summarily disqualify the bidder.
• The Hospital Management reserves the right to annul or cancel the bidding process at any time without incurring any liabilities and assigning any reason thereto.
