The Federal Medical Center, Keffi Nasarawa State invites reputable and competent consultants to submit EOI documents to tender for the following works:




The Federal Medical Centre, Keffi Nasarawa State is in the provision of efficient and effective healthcare delivery in the state and its environs, in compliance with the requirements of the Public Procurement Act, 2007 and Extant Regulations of Government on the Procurement of Goods, Works and Services intends to implement some Capital Projects through funding from the 2021 budget appropriation.

Preamble
Experienced and competent Consultants are invited to submit Expression of Interest (EoI) for the Consultancy Services while Contractors are invited to collect and submit bids for Goods and Works.

Supply, Installation and Training on the following  Medical Equipment:
  • Lot B1 / Anaesthesia machines, suction machines, blood and infusion warmer, cell server machine, nerve stimulators, syringe pumps
  • Lot B2/ Theatre Equipment and tools for the following specialties: 
    • Neurosurgery
    • Plastic Surgery
    • Cardiothoracic Surgery.

  • Lot B3 / Bus 2018 Ambulance
Category C / Consultancy Services

  • Lot C1 / Free Medical Outreach for the Sick
Eligibility Requirements
Interested bidders should submit following documents duly signed, paginated, separated by dividers and arranged in the order outlined here under:
  • Evidence of Certificate of Incorporation issued by Corporate Affairs Commission (CAC) including Form CAC1.1 or CAC2 and CAC7. Business Name is also acceptable for Services
  • Evidence of Company’s Income Tax Clearance Certificate for the last three (3) years (2018, 2019, 2020) valid till 31st December, 2021 with minimum average annual turnover of N50m;
  • Evidence of Pension Clearance Certificate valid till 31st December, 2021 (applicable where number of staff is at least 15);
  • Evidence of Industrial Training Fund (ITF) Compliance Certificate valid till 31st December, 2021 (applicable where number of staff is at least 5 or at least N50m turnover);
  • Evidence of current Nigeria Social Insurance Trust Fund (NSITF) Clearance Certificate valid till 31st December, 2021;
  • Evidence of Registration on the National Database of Federal Contractors, Consultants and Service Providers by submission of Interim Registration Report (IRR) expiring 31st December, 2021 or valid certificate issued by the BPP;
  • Sworn Affidavit:
    • Disclosing whether or not any officer of the relevant committee of Federal Medical Centre, Keffi or the Bureau of Public Procurement is a former or present Director, shareholder or has any pecuniary interest in the bidder and to confirm that all information presented in its bid are true and correct in all particular;
    • That none of the directors of the company has been convicted by any court for any criminal offence involving financial impropriety; an
    • That the company is not in any receivership, insolvency of bankruptcy;
  • For Consultancy Services: Evidence of firm’s Registration with relevant Medical professional bodies with current practicing licenses to practice;
  • For Motor Vehicle, only companies included in the National Automotive Design and Development Council approved list of Manufacturers of vehicles or their representatives should bid for the vehicle;
  • Letter of Authorization from Original Equipment Manufacturers listed in the approved policy of procurement of Health and Medical Equipment for Tertiary Hospitals in Nigeria for bidders;
  • Company’s Audited Account for the last three years (2018, 2019 & 2020);
  • Reference Letter from a reputable commercial bank in Nigeria indicating willingness to provide credit facility for the execution of the project when needed;
  • Company’s Profile with the Curriculum Vitae of key staff to be deployed for the project, indicating copies of their academic/professional qualifications (e.g COREN, QSRBN, ARCON, CORBON etc. for works,) and cognate experiences including photocopies of academic and professional certificates;
  • Verifiable Documentary evidence of at least three (3) similar jobs executed in the last five (5) years including letters of awards, Valuation Certificates, Job Completion Certificates and photographs of the projects;
  • For Joint Venture/Partnership, Memorandum of Understanding (MoU) should be provided (CAC, Tax Clearance Certificate, Pension Clearance Certificate, ITF Compliance Certificate, NSITF Clearance Certificate, IRR & Sworn Affidavit are compulsory for each JV partner)
  • All documents for submission must be transmitted with a Covering/Forwarding letter under the Company/Firm’s letter headed paper bearing amongst others, the Registration Number (RC) as issued by the Corporate Affairs Commission (CAC), contact address, telephone number (preferably GSM No.), and e-mail address. The letterhead paper must bear the names and nationalities of the Directors of the Company at the bottom of the page, duly signed by the authorized officer of the firm.
Collection of Tender Document (Goods and Works)
Interested companies (for supply and works) are to collect Standard Bidding Documents from the Head of Procurement unit with evidence of payment of non-refundable fee of N10, 000.00 only per Lot, paid into in any commercial bank with:

Opening of Technical Bids
The Technical bids will be opened immediately after the dead line for submission of tender documents (Goods and Works) stated below at the Board room of the Federal Medical Centre, Keffi, while the Financial Bids will be kept un-opened. 

Please, ensure that you sign the Bid submission register in the office of the head of Procurement Unit (office) at the administrative building, Federal Medical Centre, Keffi, old Akwanga Road, Keffi Nasarawa State as the Federal Medical Centre, Keffi will not be held liable for misplaced or wrongly submitted bids.

Opening of EOI Document
The EoI document will be opened immediately after the deadline for submission of EOI Documents in the Board room.

Submission of Tender Documents for Goods and Works
Prospective bidders are to submit bid for each lot desired two (2) hard copies each of the technical/financial bids with softcopy of financial bid only in MS Excel format by dividers and arranged as indicated above, packaged separately in sealed envelopes and clearly marked as "Technical Bid" or "Financial Bid". Thereafter, put the two sealed envelopes together in a bigger sealed envelope addressed to:

Keffi, Nasarawa State. 
And clearly marked with the name/title of the project. 

Furthermore, the reverse of each sealed envelope should have the name and address of the bidder and dropped in the designated tender box at the Procurement Unit (office) at the:

Keffi Nasarawa State.
Within official working Days/Hours (8am – 4pm) not later than closing date.

Interested firms are to submit two (2) bound of Expression of Interest (EoI) documents separated by dividers and arranged as indicated above. The documents should be submitted in a sealed envelope and addressed to the:

General Information
  • Bids/EOI must be in English language and signed by an official authorised by the Bidder;
  • Bids/EOI submitted after deadline for submission would be returned un-opened;
  • Bidders should not Bid for more than one (1) Lot;
  • All cost will be borne by the Bidders;
  • Presence of bidders/representatives and members of the public shall not be required during the bid opening ceremony in compliance with COVID-19 Guidelines. The Bids Opening will be covered by video recording (link will be sent to bidders) with the presence of two (2) representatives of private sector professional bodies and Civil Society Organisation in the areas of Anti-Corruption/Transparency;
  • Bid documents can also be collected in soft copies and Bidders can submit through Post (Courier Services);
  • Goods & Works: Only pre-qualified bidders at technical evaluation will be invited at a later date for financial bids opening, while the financial bids of un-successful bidders will be returned un-opened
  • Services: Only shortlisted firms will be invited at a later date for collection of Request for Proposals;
  • The Federal Medical Center, Keffi is not bound to pre-qualify/shortlist any bidder and reserves the right to annul the Procurement process at any time without incurring any liabilities in accordance with Section 28 of the Public Procurement Act 2007.
