Jhpiego Corporation, an affiliate of John Hopkins University, is an international non-profit, nongovernmental organization that works to improve the health of women and families throughout the world.

Applications are invited for:

Title: Request for Quotation for the Setting up of Iwoye Facility

Preamble

Jhpiego Corporation is seeking for reputable vendors to provide services below in setting up Iwoye office at Ondo state on behalf of Jhpiego Abuja. Vendors are encouraged to read the entire document to ensure that they have a clear understanding of Jhpiego requirements before submitting a bid.

Scope of Work

Item Description:

Lot	Specification	Quantity
Lot 1	Construction of perimeter fencing of 130ft by 140ft with a height of 10 coaches. To be reinforced by interval block pillars. Front view should be see -through dwarf wall while the remaining 3 side should be fully covered. The inside of the fence and front view should be plastered and painted. Detailed quantifies and prices of all materials to be used should be included in quotation. Floor Tiling: Total area 66.61sqm (4 rooms, the passage way and the front corridor). Replacement of door (2 units) - “Single Leaf Flush Door (standard Size)”.	1
Lot 2	Borehole Drilling Geophysical survey and report Drilling of borehole, casing and commissioning Provision of subversive pump Construction of overhead tank base of 15ft high for two tanks of 2000 liters capacity Provision of two units of storage plastic tanks of 2000 liters capacity each Connection of water to the building at a distance of 40ft from the water point Plumbing work (Installation of 4 wash hand sinks and 4 water point at the fence) Repairs of two toilet seats and construction of water to the toilet	1

Quotations should Include the Following for Both Categories of Equipment

Proof or license to offer required services as given by the responsible regulatory body (if applicable).
Lead-time for completion of job
Quote validity of at least 60 days.
Payment terms -Within 30 days of satisfactory job completion.

Contract Duration:

At the successful delivery of all items as on the approved Purchase Order.

Eligibility Requirements

All applicants should provide the following;

Certificate of registration/incorporation, memorandum and articles of association and valid Tax Identification Number (TIN).
Vendor must be a licensed/authorized vendor for services to be rendered (please provide supporting documentation).
Company profile (briefly reflecting the core competencies, key management personnel and clearly stating the physical verifiable office address, mobile number and e-mail address).
Proof or license to offer required services as given by the appropriate regulatory body (if applicable).
Minimum of three (3) reputable references active within the last three years form clients/organizations -
Note that applicable government taxes will be deducted prior to making payments for items supplied.
