Preamble

The Federal Medical Center Birnin Kudu intends to procure works, goods and services under the 2021 financial year in accordance with the Public Procurement Act 2007. Accordingly, interested/reputable and competent contractors/supplies are invited to submit quotation for the below stated projects.

Scope of Work

Name of Project / Description:

Lot A: Construction of Basic Pediatric Clinic/Ward at Hadejia General Hospital, Jigawa State North East Senatorial District / Details as contained in the Standard Bid Document.
Lot B: Supply and Installation of Hospital Medical Equipment's / Details as contained in the Standard Bid Document

Eligibility Requirements

Company profile this should contain among others:

Evidence of Certificate of incorporation issued by the Corporate Affairs Commission (CAC) including Forms CAC2 and CAC7 for CAC 1.1).
Evidence of Company Income's Tax Clearance Certificate for the last three (3) years valid till 31st December, 2021.
Evidence of current Pension Compliance Certificate valid til 31st December, 2021.
Evidence of current Industrial Training Fund (ITF) Compliance Certificate valid till 31st December, 2021.
Evidence of current Nigeria Social Insurance Trust Fund (NSITF)Compliance Certificate valid til 31st December, 2021.
Evidence of Registration on the National Database of Federal Contractors, Consultants and Service Providers by submission of Interim Registration Report (RR) expiring on 31/12/2021 or valid Certificate issued by BPP.
Sworn Affidavit disclosing whether or not any officer of the relevant committees of Federal Medical Centre, Birnin Kudu or the Bureau of Public Procurement is a former or present Director, shareholder or has any pecuniary interest in the bidder and to confirm that all information presented in its bid are true and correct in all particulars.
Company's Audited Accounts for the last three (3) years - 2018, 2019, 2020.
Evidence of financial capability to execute the project by submission of Reference Letter from a reputable commercial bank in Nigeria, indicating willingness to provide credit facility for the execution of the project when needed.
Company's Profile with the Curriculum Vitae of Key Staff to be deployed for the project, including copies of their Academic Professional qualifications in case of construction: COREN, QSRBN, ARCON, CORBON etc.).
Verifiable documentary evidence of at least three (3) similar jobs executed in the last five (5) years including Letters of Awards, Valuation Certificates, Job Completion Certificates and Photographs of the projects.
Evidence of dealership or manufacturer's authorization in accordance with the approved policy. (for lot B).
Prove of service location within the country and maintenance engineers for heavy Medical equipment (for lot B).
Evidence of maintenance and user training on the use of equipment.
All documents for submission must be transmitted with a Covering/Forwarding letter under the Company/Firm's Letter Head Paper bearing amongst others, the Registration Number (RC) as issued by the Corporate Affairs Commission (CAC), Contact Address, Telephone Number (preferably GSM No.), and e-mail address. The Letterhead Paper must bear the Names and Nationalities of the Directors of the Company at the bottom of the page, duly signed by the authorised officer of the firm.

Application Closing Date

12:00 noon Tuesday, 26th October, 2021.

Collection of Bid Documents

Bid documents shall be obtained at the Procurement Department, Room 3 Old Administration Block, on payment of Ten Thousand Naira (N10,000.00) only per Lotto Federal Medical Centre Jigawa, at any Commercial Bank through the Treasury single account platform.

Opening of Technical Bids

Only the technical Bids shall be opened immediately after the deadline for submission at 12:00 noon on Tuesday, 26th October, 2021, at the Hospital's conference Hall, while financial bids will be kept un-opened.

Submission of Bid Documents

Completed Technical and Financial bids in separate envelopes shall be enclosed in a large envelope labelled "Technical and Financial bids" with the project Name at the top right hand corner and the company Name behind and addressed to the:

PMB 1022, Birnin Kudu,

Jigawa State.

The documents should be deposited in the Tender Box at the Head of Procurement office, Old Administration Block, Room 3 on or the above stated closing date.

General Information

Bid must be in English Language and signed by an official.
Bids submitted after the deadline will be returned unopened.
Proof of payment of Tender fee must be attached.
Bidders are excused from the bid opening, which will be attended by officials of private sector professional organisation and CSOs in the areas of anti-corruption/transparency. The event will be fully captured on video.
Standard Bidding Documents can also be collected in Soft copies and Bidders can submit through Post (Courier Services).
Presence of Bidders/representative are not required to comply with Covid 19 guidelines the exercises will be fully capture in video coverage. Only representative from civil societies organisation and anti-corruption transparency members are required to witness the exercises.
Bidders cannot bid for more than one lot at a time.
The FMC Birnin Kudu is not bound to shortlist/pre-qualify any bidder and reserve the right to annul procurement process at anytime without incurring any liability in accordance with Section 28 of the Public Procurement Act 2007.
