Plan International Nigeria invites competent and reputable vendors to indicate interest by submitting RFP documents for the following works:

Title: Request for Proposal (RFP) for Printing of Client Intake Card and Enrollment Register

Preamble
Plan International is committed to obtaining fair value for services rendered with a competitive price and timely delivery of the right quantities at the right quality. It is the intent of this RFPs to secure competitive proposal for Printing of client in-take cards and enrollment register.

Scope of Work
S/No. / Items / Quantity.:
  • A Client intake card and Maternal client intake card / 120,000 Pieces
  • Beneficiary enrollment register / 900 Pieces
Specifications of the items:
  • Beneficiary Enrollment Register:
    • The Register will be hardcover bound 50 sheets of size A3
    • There will be 100 pages in the register.
    • Black and white printing will be done both on the front and back of each A3 sheet.
    • The enrollment tool will be on the first 80 pages
    • The commodity management tool will occupy pages 81 to page 98
    • The instructional manual will occupy pages 99 and 100
  • Client Intake Card Per Beneficiary:
    • The client intake cards will be on size A5 hard water-resistant paper that allows the use of pen or biro
    • We will have two types of client intake cards:
      • The Maternal client intake card
      • The Child client intake card
  • Both the Maternal client intake cards and the Child client intake cards will have the same biodata page at the front of the card and the back will contain the beneficiary schedule unique to the maternal client intake card and child intake card.
  • Quantity for Maternal client intake cards will be 48,000 copies while we will need 72,000 copies for the Child client intake cards.
Vendor Requirement for Pre-qualification




  • Company Profile including names and contact details of key personnel
  • Evidence of valid registration with corporate affairs commission with the (1) Copy of Memorandum and Article of Association (2) Copy of forms CAC2 and CAC7.
  • Evidence of tax clearance certificate for the last three years Tax Identification Number (TIN)
  • Evidence of corporate office, phone number and email address
  • Verifiable list of previous/similar projects/work carried out in the recent past with submission of Purchase orders or contracts as evidence
  • Evidence of financial capability and bankers’ support letters
  • Evidence of product registration with NAFDAC
  • Evidence of composition of products
  • Evidence of certification of quality of products
  • Evidence of all drugs supplied will have at least 18 months shelf life.
  • Verifiable list of previous/similar projects/work carried out in the recent past
  • Company audited account in last three years
  • Evidence of registration with Pharmaceutical Council of Nigeria or any other regulatory body.
  • Evidence of OEM where applicable.
  • Quotation validity period should also be stated.

  • Bids submitted after the deadline will NOT be accepted.

A completed Tender Should contain the following:
  • Completed Quotations for the Nutrition products, commodities and pharmaceuticals.
  • Availability to supply the items within 20 days, if chosen. (Any exceptions should be clearly stated)
  • Company Profile with the supporting documents
