Family Health International (FHI 360) invites interested companies, firms and organizations who wish to be pre-qualified to express interest by submitting EOI documents for the following works:

Title: Expression of Interest (EOI) for Pre-Qualification of Vendors for Participation in Limited Competitive Procurement Activities

Preamble
FHI360 Nigeria Country Office is seeking to prequalify interested companies/firms/organizations that met the below pre-qualification requirement. Successful applicants shall have the opportunity to participate in our subsequent limited competitive procurement method.

Pre-Qualification Requirements

Cover Page Letter on company letterhead - the subject should be indicating the category(ies) of service seeking pre-qualification for The letterhead should have the company registered address(s), official, functional emails, telephone numbers and all point of contacts for the company.
Evidence of statutory registration document of the company (CAC registration certificate, FORM CO7 and FORM CO2)
Tax Clearance Certificate in the last three years (2020, 2019 & 2018)
Evidence of Financial Capability (Latest Audited Accounts).
Bank Reference Letter duly addressed to FHI360 Pre-qualification Committee.
Evidence of Registration with Professional bodies, Regulatory Bodies, Manufacturers and/or Certificate of Authorized dealership from Original Equipment Manufacturer/other companies.
Evidence of possession of experience in the line of field/category you are seeking pre-qualification. The evidence should include copies of completed Job Orders and Proof of Deliveries (Delivery Notes or Job Completion Notification/certificate.
Three reference letters from three different organizations you have currently or had transaction with in the past attesting to your performance.
Any other relevant supporting document that could aid your application.

Evaluation Stages and Selection Criteria

Stage One: Conformity to the tender - Proposal must have all documents listed in Requirements (above). Evaluators will not make inference and assumption for incomplete information or document.
Stage Two: Review of the evidence of past experience(s) to ensure its matches the category of service applied for. Successful debarment search using UN, OFAC and SAM Searches
Stage Three: Successful physical verification and Reference Check.

Clarification Guidance

Interested applicants that require clarification should submit their query(ies) to: quotations.Nigeria@ng.fhi360.org and IHANN_Procurement@fhi360.org not later than 5:30pm on Wednesday February 9th, 2022.

Issuance of this EOI in no way constitutes a commitment by FHI360 to any Applicant.
FHI360 reserves the right to vary any part of this EOI at the time of award if it becomes necessary.
FHI360 is Value added Tax (VAT) exempted, but carryout Withholding Tax (WTH) deduction on commercial transactions in compliance with relevant Nigerian laws.
