The Federal Medical Centre, Yenagoa invites Interested, competent, and reputable contractors to submit their Tender bids for the Digitalization of Service Delivery (Procurement of Accessories). Interested bidders should possess relevant qualifications and experience.




The Yenagoa General Hospital, established in 1957 during the colonial era, was converted into a specialist hospital in 1996, after the creation of Bayelsa State. On 9th September, 1999 the same hospital was taken over by the Federal Ministry of Health and re-named Federal Medical Centre, Yenagoa. In 2014, the Otuoke Cottage Hospital established under the auspices of the MDGs was taken over by the Federal Ministry of Health following a Presidential Mandate and handed over to become Federal Medical Centre Yenagoa Outreach Hospital in Ogbia L.G.A. Today, the hospital is best described as Federal Medical Centre, Bayelsa State.

Introduction
  • In compliance with the requirements of the Public Procurement Act, 2007 and extant regulations on the Procurement of Goods, Works and Services, the Federal Medical Centre, Yenagoa, Bayelsa State, hereby invites Interested, competent and reputable contractors with relevant experience, requisite expertise and good track record to tender for the under-listed 2024 capital projects.
Eligibility Requirements
  • Evidence of Certificate of Incorporation with the Corporate Affairs Commission (CAC) including Forms CAC1 and CAC2 or CAC 7.
  • Evidence of Company's Income Tax Clearance Certificate for the last three (3) years valid till 31st December, 2024;
  • Evidence of Pension Clearance Certificate valid till 31st December, 2024 (this requirement is only applicable to bidders whose number of staff is 15 and above);
  • Evidence of Industrial Training Fund (ITF) Compliance Certificate valid till 31st December, 2024 (this requirement is only applicable to bidders whose number of staff is 5 and above or the bidder's annual turnover is N50m and above);
  • Evidence of Nigeria Social Insurance Trust Fund (NSITF) Clearance Certificate valid till 31st December, 2024;
  • Evidence of Registration on the National Database of Federal Contractors, Consultants and Service Providers by submission of Interim Registration Report (IRR) expiring on 31st December, 2024; or valid Certificate issued by BPP.
  • Sworn Affidavit disclosing whether or not any officer of the relevant committees of the Federal Medical Centre, Yenagoa or the Bureau of Public Procurement is a former or present Director, Shareholder or has any pecuniary interest in the bidder and to confirm that all information presented in its bid are true and correct in all particulars;
  • Company's Audited Accounts for the last three (3) years-2021, 2022, 2023;
  • Reference Letter from a reputable commercial bank in Nigeria, indicating willingness to provide credit facility for the execution of the project when needed;
  • Verifiable documentary evidence of at least three (3) similar jobs (in size, nature & complexity) executed in the last five (5) years including Letters of Awards, Valuation Certificates, Job Completion Certificates and Photographs of the projects;
  • Letter of Authorisation as representative of the Original Equipment Manufacturers (OEMs);
  • For Joint Venture/Partnership, Memorandum of Understanding (MoU) indicating Lead Partner should be provided (CAC, Tax Clearance Certificate, Pension Clearance Certificate, ITF Compliance Certificate, NSITF Clearance Certificate, IRR & Sworn Affidavit are compulsory for each JV partner)
Note: All documents for submission must be transmitted with a Covering/Forwarding letter under the Company/Firm's Letter Head Paper bearing amongst others, the Registration Number (RC) as issued by the Corporate Affairs Commission (CAC), Contact Address, Telephone Number (preferably GSM No.), and e-mail address. The Letterhead Paper must bear the Names and Nationalities of the Directors of the Company at the bottom of the page, duly signed by the authorised officer of the firm.

Collection of Tender Documents
  • Interested Companies are to collect Standard Bidding Document (SBDS) from the office of the Procurement Unit, Prof. E. N. Etebu Administration Block (2 Floor), Federal Medical Centre, Yenagoa within the working hours of 8am - 4pm (Monday - Friday) on evidence of payment of non-refundable Tender Fee of N10,000.00 only per LOT, paid into the Federal Medical Centre, Yenagoa's TSA at any reputable Commercial Bank in Nigeria
Application Closing Date
Tuesday; 26th November, 2024 (12:00noon).

Submission of Tender Documents
Prospective bidders are to submit bid for each of the Lot desired, two (2) hard copies the technical and financial bids with softcopy of the financial bid only in MS Excel format, packaged separately in sealed envelopes and clearly marked as "Technical Bid" and "Financial Bid" Thereafter, put the two sealed envelopes together in a bigger sealed envelope with the company name and address of the bidder indicated at the back of the envelope and addressed to: The Federal Medical Centre Yenagoa, Bayelsa. The envelope should then be dropped in the designated Tender box at the address above not later than 12:00 noon on Tuesday, 26 November, 2024.

Opening of Technical Bids
  • Only the Technical bid will be opened immediately after the deadline for submission at 12:00 noon Tuesday, 26th November, 2024 in the Boardroom of Federal Medical Centre, Yenagoa, in the presence of bidders and their representatives while the Financial bids shall be kept unopened.
  • Bidders should ensure to sign the bid submission register in the Procurement Unit, Prof. E. N. Etebu Administration Block (2nd Floor), Federal Medical Centre, Yenagoa, as the Federal Medical Centre, Yenagoa shall not be held liable for misplaced or wrongly submitted bids.

General Information
  • Bids submission must be in English Language and signed by an official authorised by the bidder
  • All costs will be borne by the bidders
  • Bids received after the deadline for submission shall be retumed un- opened
  • Evaluation of Bids and documents shall be strictly based on the requirements specified above
  • The Technical Bids will be opened in the presence of bidders or their representatives, CSOs in the areas of Anti- Corruption and Transparency and Private Professional bodies
  • Only pre-qualified bidders at technical evaluation shall be invited at a later date for financial bids opening, while the financial bids of unsuccessful bidders shall be returned unopened
  • The Federal Medical Centre, Yenagoa is not bound to pre-qualify any bidder and reserves the right to annul the Procurement process at any time without incurring any liabilities in accordance with Section 28 of the Public Procurement Act 2007.
