Invitation for Pre-Qualification to Tender for Year 2013 Capital Projects at Neuropsychiatric Hospital

Preamble
Applications are hereby invited from qualified contractors to tender for prequalification for the following Projects which will be funded through 2013 Capital Appropriation.

The projects are as follows:
Lot 1: Major Rehabilitation of2-Storey, 90-Room Student Nurses Hostel.
Lot 2: Construction of six (6) Blocks of four (4) each 2-Bedroom apartments for Resident Doctors.
Lot 3: Purchase of one (1) No. 10,000litres Water Tanker.
Lot 4: Handling of Local/Overseas Training Programme for Residents’ Doctors and other Health Professionals.

Prequalification Requirements
Interested Contractors/Companies are required to submit the following documents in sequential order as listed:

• Certificate of Registration with the Corporate Affairs Commission (C.A.C).
• Company Tax Clearance Certificates for last three (3) successive years (2009, 2010 and 2011).
• Evidence of VAT Registration and Remittances
• Evidence of Certificate/Exemption from PENCOM in accordance with the Pension Reform Act (2004) including evidence of remittances of Employees Pension Contributions/Deductions
• A sworn Affidavit that no officer of Neuropsychiatric Hospital, Aro, Abeokuta is a former or present Director, or has any pecuniary interest in the bid.
• Evidence of financial capability to execute the project (e.g. Current Bank Statement certified by the company’s Banker) and/or Banking support.
• Verifiable list of similar jobs successfully executed in the last three (3) years including letters of award of contracts, project costs, Job completion certificates etc.
• Company’s Audited Account for 2009, 2010 and 2011
• Company’s profile including Professional Qualifications of key personnel and equipment (where applicable),
• Any other additional information that may enhance the company’s application.

Note:
Documents (a) - (e) are RESPONSIVE criteria, the absence of any automatically disqualifies the bidder for pre-qualification.

Closing Date/Submission of Pre-Qualification Documents

• The completed pre-qualification documents are to be submitted in sealed envelope with the name and address of the prospective bidder at the top left corner of the envelope with tenderer’s GSM Number/Reachable number.
• All documents should be submitted within six (6) weeks i.e. on or before 9:00am on Wednesday. 20th February, 2013 when documents will be opened.

• The documents must be dropped in the Bid Box located in the office of the Provost and Chief Medical Director.
• Tenderers should note that only prequalified companies/contractors would be invited to submit financial bids. The hospital reserves the right to reject any or all prequalified documents.

All Tenderers and interested stakeholders are invited to the Tender opening scheduled for 10.00am on Wednesday, 20th February, 2013 in the Provost and Chief Medical Director’s Boardroom.
