Edo State Essential Drugs Programme Prequalification for the Supply of Essential Drugs/Dressings

The Edo State Essential Drugs Programme through the Ministry of Health, Benin City, wishes to procure Essential drugs/dressings from reputable pharmaceutical companies (major manufacturers and distributors). Interested companies are hereby invited to apply for pre-qualification to participate in quarterly bidding for the supply of Essential drugs/dressings (Please note that only prequalified companies will be invited to participate in tendering for 2013).

Pre-qualification Requirements:
Interested companies are expected to submit the following documents for pre-qualification:

• Evidence of registration of the company with the Corporate Affairs Commission;
• Evidence of up-to-date registration with the Pharmacists Council of Nigeria;
• Evidence of registration of the Superintendent Pharmacist with the Pharmacists Council of Nigeria;
• Evidence of registration with the Edo State Government (Essential Drugs Programme (EDP);
• Evidence of payment of taxes to the Edo State Government;
• Evidence of settlement of PAYE;
• Evidence of similar completed jobs within the last three (3) years;
• Evidence of Corporate Tax Clearance Certificate for the last three (3) Years;
• Company’s Profile stating: contact address including GSM number, e-mail address and website;
• Evidence of financial capability.

Submission
Pre-qualification documents are to be addressed in sealed envelope and submitted to the Programme Manager, Edo State Essential Drugs Programme, Medical Stores Complex, Medical Stores Road, Benin City
